Bayhealth, Inc. is seeking motivated and detail-oriented individuals for our Home Health Scheduling Coordinator. In this role, you will play a pivotal role in organizing and maintaining clinicians’ schedules, ensuring efficient allocation based on patients’ needs. This role involves effective communication with clinicians and collaboration with various teams.

Responsibilities:

• Create and maintain clinician schedules based on authorizations, patient needs, and clinician availability.
• Ensure staffing schedules provide adequate coverage for sick leave, vacation, and long-term leave while liaising between the Intake team and Clinical team.
• Serve as the primary point of contact for interactions with clinical team members, administrative associates, referral sources, and patients.
• Partner with clinical leadership to direct the coordination of care and maintain patient schedules in alignment with care plans and staff availability.
• Review physician orders and authorizations, schedule appropriate skilled clinicians, communicate routinely with clinical supervisors, oversee new case staffing, and manage staff and patient census reports.

Qualifications;

• High School diploma or equivalent (GED).
• Preferably an Associate's/Technical Degree or a comparable combination of education and related experience.
• Minimum one year of experience in home health is preferred with a demonstrated ability to engage in cross-functional collaboration.
• Demonstrates strong organizational and interpersonal communication skills.
• Thrives in a self-directed work environment, excelling in multitasking, and actively contributes to cross-functional teams.
